Delirium
An acute, reversible state of confusion usually resulting from an underlying medical condition, surgery, or medication toxicity.
Assessment Findings
- Sudden onset of cognitive impairment and disorientation
- Fluctuating level of consciousness throughout the day
- Inability to sustain focus or attention
- Visual hallucinations and disorganized thinking
Nursing Priorities
- Identify and treat the underlying cause (e.g., infection, dehydration)
- Implement safety measures to prevent falls
- Reorient the patient frequently with clocks and calendars
- Use non-pharmacological interventions for agitation before sedation
