Weight-Based Dosing
Calculation methods used when medication dosage is determined by the patient's body weight, common in pediatrics and critical care.
Standard Procedure
- Convert patient weight from pounds to kilograms (1 kg = 2.2 lbs).
- Multiply the ordered dose (mg/kg) by the patient's weight in kg to find the total dose.
- Use the 'Have/Have' method to find the volume if the medication is in liquid form.
Nursing Precautions
- Double-check all decimal point placements.
- Verify the safe dose range for the specific medication if provided.
- Always re-weigh the patient if the clinical assessment contradicts the current weight on file.
