Heparin Infusion Titration
Calculating heparin drip rates based on weight and titrated based on PTT/anti-Xa lab values.
Initial Dose
- Calculate based on the ordered units/kg/hr.
- Confirm the concentration of the bag (e.g., 25,000 units in 500 mL).
- Set the pump to the mL/hr equivalent of the prescribed unit dose.
Titration Guidelines
- Adjust the infusion rate based on the institutional nomogram.
- Monitor PTT or anti-Xa levels at prescribed intervals, usually every 6 hours after dose changes.
- Assess for signs of bleeding: hematuria, gingival bleeding, bruising, or drop in hemoglobin.
