Umbilical Cord Prolapse
An obstetric emergency where the umbilical cord slips down in front of the presenting part of the fetus.
Assessment
- Visible or palpable cord at the vaginal opening
- Sudden fetal bradycardia or recurrent variable decelerations
- Umbilical cord pulsation detected during digital exam
Immediate Nursing Actions
- Call for assistance immediately
- Apply upward pressure on the presenting part with a sterile gloved hand to lift off cord
- Reposition mother to Trendelenburg or knee-chest position
- Prepare for emergency cesarean section
Patient Care
- Administer O2 at 8-10L/min via non-rebreather mask
- Maintain constant fetal heart monitoring
- Do not attempt to push the cord back into the uterus
