Chronic Kidney Disease (CKD)
Progressive, irreversible loss of kidney function characterized by a decrease in glomerular filtration rate.
Assessment Findings
- Elevated BUN and Creatinine
- Fluid retention and hypertension
- Anemia due to decreased erythropoietin
- Electrolyte imbalances (Hyperkalemia)
Nursing Management
- Monitor intake and output strictly
- Restrict potassium and phosphorus intake
- Provide skin care to reduce pruritus
- Assess for signs of fluid overload
- Educate on dialysis vascular access care
