Warfarin Therapy

Warfarin is an oral anticoagulant that acts as a Vitamin K antagonist to prevent thromboembolic events.
Monitoring
- Monitor PT/INR levels regularly; target INR is typically 2.0-3.0.
- Vitamin K is the antidote for warfarin overdose.
- Monitor for signs of bleeding: hematuria, epistaxis, gum bleeding, bruising.
- Instruct patient to maintain consistent intake of Vitamin K-rich foods (leafy greens).
Patient Education
- Use a soft-bristled toothbrush and electric razor.
- Avoid NSAIDs and aspirin due to increased bleeding risk.
- Notify provider before any dental or surgical procedures.
