Adhering to the 'Rights of Medication Administration' is the primary defense against medication errors.
The 6 Rights
- Right Patient: Use two unique identifiers.
- Right Medication: Compare order to label.
- Right Dose: Verify calculations carefully.
- Right Route: Do not switch routes (e.g., oral vs. IV).
- Right Time: Adhere to scheduled intervals.
- Right Documentation: Sign immediately after administration.
Safety Checks
- Check allergies prior to administration.
- Clarify illegible or questionable orders with the provider.
- Never administer a medication prepared by someone else.
- Observe the patient swallow oral medications.
