Fall Risk Assessment
Nurses must systematically evaluate patients for fall risk to implement preventative measures.
Risk Factors
- Age 65 or older.
- History of previous falls.
- Impaired gait or mobility.
- Polypharmacy or sedative medications.
- Cognitive impairment or confusion.
Prevention Strategies
- Keep bed in the lowest position with wheels locked.
- Place call light within immediate reach.
- Provide nonslip footwear.
- Clear pathways of obstacles and clutter.
- Use bed alarms for high-risk patients.
