A disorder featuring intrusive, distressing thoughts (obsessions) and ritualistic behaviors (compulsions).

Clinical Features

  • Repetitive, unwanted thoughts
  • Ritualistic behaviors to reduce anxiety
  • Recognition that thoughts are unreasonable but uncontrollable
  • Significant distress and impairment in functioning

Nursing Management

  • Allow time for rituals early in treatment to decrease anxiety
  • Gradually structure a schedule to limit ritual time
  • Encourage participation in new activities
  • Teach relaxation techniques like deep breathing

Priority Actions

  • Focus on patient safety during ritual performance
  • Avoid reinforcing the rituals
  • Establish a trusting relationship to encourage open communication
