Anticoagulation: Heparin Therapy
Heparin is a rapid-acting anticoagulant used to prevent thrombus formation by potentiating antithrombin III.
Laboratory Monitoring
- Monitor Activated Partial Thromboplastin Time (aPTT) to adjust dosage.
- Therapeutic range is typically 1.5 to 2.5 times the control value.
- Check baseline and periodic platelet counts to monitor for Heparin-Induced Thrombocytopenia (HIT).
Safety & Administration
- Antidote for overdose is Protamine Sulfate.
- Use soft-bristled toothbrush and electric razor to minimize bleeding risk.
- Do not rub the injection site after subcutaneous administration to avoid hematoma.
