Assessment

  • Decreased capillary refill and skin turgor
  • Sunken fontanel in infants
  • Lack of tears when crying
  • Dry mucous membranes and decreased urine output

Rehydration Plan

  • Oral Rehydration Solution (ORS) for mild to moderate cases
  • Isotonic IV fluids (e.g., 0.9% Normal Saline) for severe dehydration
  • Strict monitoring of weight and vital signs
  • Avoid carbonated or high-sugar drinks
