Apgar Scoring
A quick assessment of the newborn's physical condition performed at 1 and 5 minutes after birth.
Scoring Categories
- Appearance (color)
- Pulse (heart rate)
- Grimace (reflex irritability)
- Activity (muscle tone)
- Respiration (effort)
Interpretation
- 0-3: Severe distress
- 4-6: Moderate difficulty
- 7-10: Stable condition
Nursing Documentation
- Document scores precisely at 1 and 5 minutes
- If the 5-minute score is < 7, continue assessing every 5 minutes until 20 minutes of age
