Postpartum Hemorrhage

Excessive bleeding following childbirth, defined as blood loss > 500mL for vaginal birth or > 1000mL for cesarean birth.
Risk Factors
- Uterine atony (most common cause)
- Grand multiparity
- Prolonged labor
- Overdistended uterus (polyhydramnios, twins)
Immediate Nursing Actions
- Massage the fundus firmly
- Assess bladder for distension; empty the bladder if necessary
- Administer oxytocin, methylergonovine, or carboprost as ordered
- Monitor vital signs and pad count for blood loss quantification
Signs of Shock
- Tachycardia
- Hypotension
- Cool, clammy skin
- Mental status changes
