Levels of Evidence Hierarchy

The hierarchy of evidence ranks research studies based on their clinical rigor and potential for bias to inform Evidence-Based Practice (EBP).

Hierarchy Tiers

  • Level I: Systematic reviews or meta-analysis of randomized controlled trials (RCTs).
  • Level II: Single well-designed randomized controlled trial.
  • Level III: Controlled trials without randomization (quasi-experimental).
  • Level IV: Case-control or cohort studies.
  • Level V: Systematic reviews of descriptive or qualitative studies.
  • Level VI: Single descriptive or qualitative study.
  • Level VII: Expert opinion or consensus reports.

Clinical Application

  • Use Level I-III evidence for clinical decision-making when possible.
  • Higher levels of evidence provide the strongest recommendations for nursing interventions.
  • Always evaluate the quality and currency of the source regardless of level.
