Delirium
An acute, reversible state of confusion, often triggered by a medical condition or medication toxicity.
Assessment Findings
- Rapid onset of confusion
- Fluctuating levels of consciousness
- Disorientation to time and place
- Visual hallucinations possible
- Inattention and disorganized thinking
Nursing Interventions
- Identify and treat the underlying medical cause
- Provide reorientation frequently
- Ensure a safe, quiet environment to reduce sensory overload
- Maintain circadian rhythm with consistent sleep-wake cycles
Nursing Priorities
- Maintain patient safety to prevent falls
- Avoid physical restraints if possible
- Document specific changes in cognitive status
