Nasogastric (NG) Tube Insertion
Insertion of a flexible tube through the nasopharynx into the stomach for decompression, enteral feeding, or medication administration.
Pre-procedure
- Verify order and explain procedure to patient.
- Position patient in high Fowler's position.
- Measure tube from tip of nose to earlobe, then to xiphoid process.
- Lubricate tip with water-soluble lubricant.
Insertion and Verification
- Instruct patient to swallow or sip water during advancement.
- Check for respiratory distress or coughing.
- Confirm placement with X-ray (gold standard) or pH testing of aspirate (pH < 5).
- Secure tube to nose with tape or device.
Nursing Maintenance
- Check tube placement before every feed or medication.
- Flush tube with water regularly to prevent clogging.
- Monitor for skin breakdown at the nare.
- Maintain head of bed at 30-45 degrees to prevent aspiration.
