The final phase of the nursing process where the nurse assesses if the interventions effectively resolved the diagnosis.

Evaluation Indicators

  • Met: The goal was achieved; discontinue or update the plan.
  • Partially Met: The goal is in progress; adjust interventions.
  • Not Met: Reassess the patient, etiology, and interventions for errors.
  • Documentation: Always state the specific reason for the outcome status.
