Pressure Injury Prevention

Geriatric skin is prone to breakdown due to decreased subcutaneous fat and thinning of the epidermis.
Risk Mitigation
- Turn and reposition immobile patients at least every 2 hours.
- Maintain head of bed at 30 degrees or less to prevent shear.
- Keep skin clean, dry, and apply barrier creams for incontinence.
- Use pressure-redistributing mattresses or cushion surfaces.
Nutritional Support
- Ensure adequate protein intake for tissue repair.
- Maintain hydration levels to promote skin elasticity.
- Assess serum albumin and prealbumin levels periodically.
